    Contact your Benefit Liaison to request Arizona Foundation provider directories or call Arizona Foundation at (602) 252-4042 or (800) 624-4277 (outside of Maricopa County).

  • How do I find out if my claim has been paid?
    Call Harrington Benefits, your Plan Administrator, at (888) 999-1459.

  • Who do I contact to obtain a precertification (or preauthorization) for hospital admissions?
    Physicians or individuals can request precertification by calling (888) 999-1459.

  • How can I nominate my doctor for Arizona Foundation membership?
    Please contact Arizona Foundation Network Management at (602) 495-8699 or (800) 624-4277 (outside of Maricopa County). Or, you can complete the Provider Nomination Form located on this website.
