Established in 1969, Arizona Foundation for Medical Care is one of the oldest and largest Preferred Provider Organizations (PPO) in Arizona. We have a network of over 9,000 physicians with practices in over 16,000 locations, as well as hundreds of hospitals and urgent care centers.

Arizona Foundation is a not-for-profit organization governed by a voluntary Board of Trustees which makes us unique. Because there are no stockholders or investors to pay, financial gains are re-invested into improving service to all of our customers.

Arizona Foundation Offers:

• Over thirty-four years experience in provider relations, claims repricing and utilization management means expertise, name recognition and stability.
• The largest, most comprehensive network in Arizona giving state employees and their covered family members the greatest flexibility when choosing a physician.
• Arizona Foundation providers are contracted on a calendar year basis and agree not to drop out of the network during the year unless they are no longer practicing in Arizona.
• Each provider undergoes a rigorous credentialing process.
