Our Unique Difference 
 
Arizona Foundation for Medical Care (AFMC) was founded in 1969 as a not-for-profit organization by a group of 800 physicians from the Maricopa County Medical Society.
These physicians believed that there was a better way to create and manage a healthcare network in Arizona.  
 
Today, AFMC has grown into Arizona’s largest statewide independent healthcare network by providing highly-accessible, quality care, while focusing on containing healthcare costs for employer groups through comprehensive medical management and wellness programs.   
 
AFMC’s unique difference, which has enabled it to thrive for nearly 40 years, is that AFMC is not a single plan or insurance company, but rather a managed care network and medical management company. Working directly with brokers, consultants, general agents, third party administrators, and insurance companies, AFMC provides Arizonans the freedom of choice by offering a variety of health plans.  
 
This flexibility enables AFMC to offer the most cost-effective network options to fit the needs of individuals, families or businesses. These plans include Foundation PPO, Foundation EPO/PPO Plus, Foundation Point of Service (POS), Foundation Select and Foundation Comp, in addition to AFMC's newest products: A parnership in VentureNet Healthcare, a Regional Preferred Provider Network (RPPO) serving employer groups in the Western half of the country; and Benefit Protect an individual health plan offered to AFMC contracted providers, their staffs and families.  
 
AFMC also offers a comprehensive package of medical management services that include: Utilization, Case and Disease Management, 24-hour Nurse Care Line, Maternity Management, plus a comprehensive Wellness Program that includes AFMC's own "Healthfull Hints" program.   
 
Many of the programs now recognized as standard benchmarks for managed healthcare plans across America were pioneered by Arizona Foundation for Medical Care. AFMC’s network includes more than 12,000 professional providers in 24,000+ locations throughout Arizona, as well as a comprehensive contracted network of  hospitals, laboratories, outpatient surgical facilities, urgent care facilities, radiology centers, and other ancillary facilities.

Cost Effective 
 
AFMC's fee schedule is determined by an independent Reimbursement Committee, comprised of employer group representatives who are elected each year from employer groups using AFMC endorsed or sponsored plans and three nonmember physicians. The Committee meets several times a year to review, amend and set procedure allowances.

Commitment to Customer Service 
 
AFMC's Call Center is locally based and our representatives respond to over 8,500 calls a month. They help our customers find the best doctor, answer their questions, or solve problems.
