Update to Arizona Foundation for Medical Care's (AFMC) Network
Gilbert Hospital’s Contract – Modification in Plan Participation Effective Jan. 1, 2008, AFMC’s contract status with Gilbert Hospital changed. Gilbert Hospital now provides services within AFMC’s: - PPO Network;
- Worker’s Comp Network; and as a
- Tier 2 “Traditional Hospital” within AFMC’s POS Network.
Members who currently receive healthcare treatment/services at Gilbert Hospital; live in the geographic area of Gilbert Hospital; and/or access AFMC’s PPO Plus/EPO Network, may utilize the following hospitals instead: - Banner Desert Medical Center in Mesa, AZ
- Banner Baywood Medical Center in Mesa, AZ
- Banner Gateway Medical Center in Gilbert, AZ
