Medalist ll PPO health plan includes:

  • Access to AFMC's network, which is the largest in Arizona. AFMC's extensive network gives beneficiaries convenient access to more than 11,000 physicians in 20,000 locations in addition to the 119 hospitals; 215 laboratories; 27 outpatient surgical centers; and 255 radiology centers statewide
  • Three different plans (Gold, Silver and Bronze).
  • Preventive care benefits of up to $1,000 annually per covered family member.
  • An accident benefit that waives the deductible and/or co-pay for covered charges incurred within 30 days of an injury.
  • A common accident benefit that applies only one deductible to covered charges if a single accident injures more than one family member.
  • Annual eye exam.
  • $5 million lifetime policy maximum.
  • Accidental death and dismemberment for the primary insured.
  • A 24-hour Nurse-line.
  • Health and wellness programs featuring Cool Tools™ and World Doc™ (an online database).
  • A comprehensive Web site that provides medical assistance for health, wellness and preventative and clinical issues allowing the end user to make more informed healthcare decisions.
  • Optional coverage is available for dental, maternity, and a $1,000 prescription drug deductible.
