PREGNANT WOMEN/HIGH RISK FLU VACCINATIONS FOR BENEFIT OPTIONS – STATE OF ARIZONA
Women who will be pregnant during influenza season are at high risk for developing complications if they get the flu. To ensure that vaccine is available for high-risk individuals, including pregnant women, Benefit Options Wellness is allowing pregnant and high-risk individuals to receive the flu vaccine before the general population.
Because we are unable to identify all pregnant women, we are asking you to spread the word. Benefit Options asks that you help us notify your pregnant patients that a free flu shot is available through the Benefit Options Wellness program.
To receive their free flu shot at these high-risk events, all pregnant women and other high-risk individuals need to either present a copy of this letter or a doctor’s note. This offer is only valid at the events listed on the website below.
Flu shot schedules are subject to change based on vaccine availability. Those who do not have Internet access, you may call Healthwaves at 480-968-1886.
To get their vaccination at these high-risk events, eligible Benefit Options members must bring the following items to these events:
A copy of this fax or a doctor’s note
Benefit Options Insurance card
Photo ID
Employee Identification Number (EIN) of the primary insured (State employee or retiree)
* All other members (non-high risk) can receive a flu shot starting in mid-October (high risk individuals are eligible to receive a flu shot any time throughout the 2006 flu vaccine program).
