New AFMC Allowables, effective August 16, 2006

Arizona Foundation for Medical Care’s Independent Reimbursement Committee has set a fee allowance for the following:

§ Human Papilloma Virus (HPV) vaccine
§ New CPT Code 90715 (Tetanua, Diptheria, Toxoids, Acellular Pertussis) vaccine
